Monitoring outcomes and measuring success

Evaluation closes the loop in the TRIP process. It helps you understand whether change has been implemented as intended, what impact it has had, and how results can be sustained and shared.

Effective evaluation starts during planning, not after implementation, and includes both process and outcome measures. This module builds on your Implementation Plan by showing how to measure what has changed and why it matters. Learn how to choose practical indicators that demonstrate whether your practice change is improving care, efficiency, and outcomes.

Logic models

Program logic models map the pathway from project initiation to impact. A program logic model illustrates how activities lead to measurable change. Program logic models are a way to:

  • Clarify structure and purpose
  • Guide implementation
  • Engage stakeholders
  • Support evaluation

Your logic model should evolve across the TRIP process – from initial planning to final evaluation. It’s important to revisit this regularly throughout your project.
